Covid Declaration Form

The Bay Medispa and team have health and hygiene best practice, policies and procedures in place to ensure the safety and wellbeing of our patients and continue to follow NSW health & safety recommendations.

We kindly request for all guests to complete a Covid Safe online declaration before arriving to their appointment and ask if anyone has been to a large event (>100 persons), or have felt unwell or in contact with a positive case within the previous 14 days to submit a negative Covid test prior to appointment.

Do you have any symptoms of covid such as fever, dry cough or tiredness?*
Have you had any close contact in the past fourteen days with any person who has tested positive for Covid?*
If you have answered yes to any of the above, you are required to email (hello@thebaymedsipa.com.au) with a copy of a negative covid test within 72 hours of your appointment.*
Please confirm that you comply and agree with our above Covid Safe Policies they are subject to constant change*
The Bay Medispa will be checking temperatures for all staff and patients on arrival.*
